Credit cards/ACH payment process is done via Docusign.

Because of the ongoing need for counseling associated with prescriptions for mental health, this is currently not covered. Contact our office if you need referrals in your area.

For emergencies, you should call 911. Urgent same day on demand visits are arranged through MyChart.

Account Information FAQs

You can print your medical records off of MyChart at no charge. Or you can request your entire record on the Docusign app/form.

The fee schedule for lab testing is on the website and on Docusign and needs to be prepaid via the app.

Phone and email information can be updated in the MyChart App.

State ID Cards or certified birth certificates are also acceptable alternatives. Passports/Visas or other government issued employment authorization cards are also acceptable alternatives.

Docusign is secure/HIPAA compliant. MyChart uses the same security system used by the banking industry.

The cancellation policy is on the Docusign portal.

No, we recommend using Good RX coupons or Walmart’s $4, $9, or $15 per month program.

General FAQs

By either calling the pharmacy it’s currently at or by taking your emailed prescription copy to the preferred pharmacy in person.

MHCO never requires a referral. If you need to be referred to a specialist we may be be able to offer one for those enrolled in the executive platinum plan.

Prescription orders will be emailed to you. Test and lab orders are available on MyChart.

Prescriptions are only provided and allowed for the patient that has been provided consultation via the prescribing physician.

The process to enroll minors can be found through docusign.

You will be emailed a copy of your prescription order when it is sent to the pharmacy. Call the pharmacy to verify it is in stock and refillable.

Visit the MyChart messenger app.

We cannot specify when to return to work or work release guidance. A work note may be provided after consultation.

No, you must print out your results and deliver to the desired recipient.

Adult dependents are enrolled through Docusign.

Medical history can be put into MyChart.

Appointment times for eVisits/audio televisits are available on MyChart.

Contact your chosen pharmacy.

Enrollment in the premium plan is required and then appointments can be scheduled.

Messages can be sent at any time to the physician. Response times are outlined in the patient enrollment agreement on docusign.
